WAIVER & RELEASE OF LIABILITY

By signing this document, I acknowledge that I have read, understood, and agreed to the terms and conditions of this Wavier and Release of Liability for participation in the FrontLine Academy Grand Opening Event and related activities.

1. ASSUMPTION OF RISK

I understand that participation in simulator-based firearms training and related activities may involve inherent risks including slips, falls, loud noises, flashing visuals, physical movement, and the use of CO2 recoil simulator equipment. I voluntarily assume all risks associated with participation.

2. RELEASE OF LIABILITY

I hereby release and discharge FrontLine Academy Firearm Training, its owners, instructors, staff, volunteers, affiliates, and property owners from any and all claims, liabilities, damages, or causes of action arising from participation in event activities, except in cases of gross negligence or intentional misconduct.

3. SAFETY RULES

I agree to follow all instructions and safety procedures provided by FrontLine Academy staff.

• Treat all simulator firearms as if they are real.

• Keep the muzzle pointed in a safe direction.

• Keep finger off the trigger until instructed.

• No horseplay or reckless behavior.

• Follow all staff instructions immediately.

4. YOUTH PARTICIPATION POLICY

Participants under the age of 18 are permitted to participate only when accompanied by a parent or legal guardian and with signed parental consent.

6. MEDICAL AUTHORIZATION

In the event of an emergency, I authorize FrontLine Academy staff to obtain emergency medical assistance if necessary. I understand that I am financially responsible for any medical expenses incurred.

9. ADULT PARTICIPANT AGREEMENT

I acknowledge that I have read and fully understand this Waiver and Release of Liability and voluntarily agree to its terms.
